Partnering for Progress: How Strategic Collaboration Can Influence Community Health
From income and job security to education and housing, social factors can be the dominant force shaping human health. These everyday conditions influence our stress, eating habits, exercise, access to care and a host of other risk factors. If we’re to improve health across the population, relationships among healthcare providers, employers, insurers and community organizations must play a foundational role. Close collaborations among these stakeholders can help patients to manage risks, clinicians to understand communities and health systems to reimagine care platforms beyond traditional settings. Join us for a panel discussion about integrating social determinants—including day-to-day behaviors—into the core of holistic community health and wellness strategies. Together we’ll explore the necessity of awareness, the promise of partnerships and technology, and the importance of personalized, community-based care in preventing adverse outcomes and strengthening population health.
Key takeaways from the session:
• Focus on addressing social determinants of health, with an emphasis on how behaviors impact these determinants at the population level, to improve healthcare outcomes.
• Discover how health systems can leverage community partnerships to build better community health and wellness.
• Learn how better community care is correlated to individual health outcomes.
